There appear to me to be three courses open for

To retain the separate appointment of Principal Civil Medical Officer as at present, on a considerably reduc- -ed salary, with a rent charge for the house.

(b).

To appoint either the Medical Officer of

Health or the Medical Officer in charge of the Government Civil Hospital as Principal Civil Medical Officer with a

Duty Pay or a "Charge Allowance", while discharging those

duties.

(c). To abolish the post of Principal Civil Medical Officer and make the Medical Officer in charge of Government Civil Hospital responsible to Government for all Hospitals and purely medical matters under the title of "Superintendent of Hospitals", while the Medical Officer of Health holding equal rank and status is held responsible direct to Government for all sanitary matters including perhaps certain questions in connection with Quarantine &c.

The first is advocated by Dr. Atkinson whose

minute I attach, but I do not myself support it for the reasons I have given. If it is adopted, I would suggest that the salary

should be £720, with the house at present occupied by Dr. Atkinson

for which a rent of X100 per mensem should be charged, and that a

Duty Pay of £150 per annum should be attached to the post.

The second course is practically identical

with that sanctioned by Mr. Chamberlain if the Superintendent of

the Government Civil Hospital is made Principal Civil Medical

Officer but a duty pay of say £120 is added. He already enjoys

free quarters and an allowance of $240 as Superintendent of Lunatic Asylum. The latter might be abolished since there is no Lunatic Asylum properly speaking (see my Despatch No. 52 of the 9th. February) being replaced by the Duty Pay. It is probable that
